Resources are available from Blue Cross Blue Shield of Michigan to help educate yourself and your employees about this chronic condition that continues to impact more and more Americans. For individuals living with chronic conditions such as diabetes, the day-to-day responsibilities of living with that condition can sometimes be challenging. Thankfully, Blue Cross and Blue Care Network work with Teladoc Health to offer condition management programs to their members. These programs offer easy-to-use tools and resources to help individuals living with conditions such as diabetes, prediabetes, and hypertension to better manage their conditions in order to live healthier, happier lives. Employers can utilize the Diabetes and Condition Management Employer Toolkit to help educate employees about the condition management resources and support available to them through Blue Cross Blue Shield of Michigan. This toolkit includes flyers, email templates, and newsletters that can be easily downloaded and used by employers to ensure they are getting the most out of their investment in their employees’ health benefits.
