This form — for managers and supervisors to use — focuses on coaching employees in positive ways. In written reprimands, the focus always is on the bad behavior. In performance reviews, the focus often is on what was wrong. This form helps you and your managers and supervisors focus your efforts — and your employee’s efforts — on the positive.
On one side, you can list any weak, wrong, or destructive traits an employee has which are causing problems in the workplace or lowering the employee’s productivity. Then focus your and the employee’s attention on what you and the employee can do to change these negative traits, actions and behaviors.
On the other side, you can list the employee’s strong and outstanding traits and actions, and the employee’s potential for greater achievement. Then under that column, you and the employee focus on what you and the employee can do to achieve more of the good.
At the very bottom is a statement for the employee to acknowledge with a signature.
